Heart Optimization Terms and Conditions.

By purchasing a spot in the heart optimization group program, you (or "Purchaser") are legally bound to the following terms:

Purchaser understands that Purchaser will be partaking in an Entirely Nourished (or the “Company”) group heart health program (“Program”). The Program is not an individualized medical nutrition therapy or counseling program. What the Program provides is included below.

Once purchased, Purchaser must fill out an intake form within 48 hours to ensure eligibility in the program. Once the intake form is completed, the Purchaser will be notified within 48 hours, unless she is out of office in which she will respond as quickly as she can, if Purchaser is a good fit for the Program and welcomed, or if Purchaser is not a good fit for the program. If you are not a good fit, Michelle will reach out to you and refund your deposit. Otherwise, all sales are final.

The Program will include the following:

1. Six weeks of live video sessions via Zoom on Friday at 12pm EST. This starts on Friday May 10, 2024 and concludes on Friday, June 14, 2024. All sessions will be recorded and provided to the Program participants to watch at their convenience, if they cannot make it live. The sessions will be available to watch until August 8, 2024. 
2. A closed Facebook group that allows for questions, peer support, and accountability.

Program Cost and Financial Policy 
The 6 week program costs $897.00 USD. Payment is required to guarantee Purchaser's place in the group and his/her commitment to the Program. The Program's coaching is not considered medically necessary and is not covered by in-network insurance.
 
All sales are final, unless Michelle deems you to not be a good candidate for the program. This is determined by filling out a short intake form after payment is received. Michelle will review this intake form within 48 hours, or if she is out of office, upon her return. If you are not a good fit, Michelle will reach out to you timely and refund your deposit. Otherwise, all sales are final.

If you do not fill out the intake form timely, your access to the program will be delayed until the intake form is completed.

No-Show and Cancellation Policy 
Group sessions may not be rescheduled, and fees are non-refundable. It is understood that Purchaser may not be able to make all of the Group sessions, and that Michelle provides all recorded sessions to Program participants to watch at their convenience.

Confidentiality
All information shared during the Group meetings is confidential. All information from each Group participant is confidential, with the exception of circumstances governed by law, such as child abuse and danger posed to self or others. All Group participant identities are confidential. All parties agree to take all reasonable measures to ensure confidentiality of Group participants and communications over the Internet.

I, Purchaser, agree to allow for recording of the group weekly meetings to occur where I may be shown and recorded on video, that can be shared to the other participants in the group via the closed Facebook group and/or participant emails.

Liability Release
By purchasing this program and clicking the box on the purchasing page, Purchaser has read and understands this contract, and agrees to the terms of this Consent Form. Purchaser acknowledges that the Company can make no guarantee or warranty as to the results of the Program. Purchaser releases the Company of all liability as to his or her personal results of this Program. Purchaser understands that his or her results may not meet his or her desires, and that weight loss and blood work improvements, as a lifestyle change, takes time. Purchaser therefore releases the Company from all liability as to his or her personal results from this Program.
